It is the line you see on every barefoot shoe website: they strengthen your feet. It is also the claim most worth being sceptical about, because "strengthen" is doing a lot of work in that sentence. Stronger how? Measured by whom? And how long does it actually take?
The good news is that this is one of the few barefoot claims with real, published research behind it — including a randomised trial and a six-month study on ordinary adults. The honest news is that the research also comes with caveats that most shoe brands quietly leave out. Here is the whole picture.
First: which muscles are we actually talking about?
Your foot has two sets of muscles. The extrinsic ones start in your lower leg and pull on the foot from a distance — your calf muscles are the obvious example. The intrinsic ones start and end inside the foot itself. They are small, they are not glamorous, and they do the quiet work: holding your arch, spreading your toes, and stabilising your foot through every single step.
A conventional shoe takes over part of that job. Arch support props up the arch. A stiff sole stops the foot flexing. A raised heel shifts your weight forward before you have even taken a step. None of this is evil — but a muscle that is not asked to work does not stay strong. That is the whole premise behind minimalist footwear, and it is a testable one.
What the research actually found
Two studies are worth knowing by name, because almost everything else you read online is a summary of one of them.
Ridge et al. (2019), Medicine & Science in Sports & Exercise. Fifty-seven runners were randomly assigned to one of three groups: walking in minimalist shoes, following a dedicated foot-strengthening exercise programme, or a control group. Over eight weeks, foot muscle strength was measured with custom dynamometers and muscle size with ultrasound. The finding: walking in minimalist shoes was as effective as doing the foot exercises. The researchers also made a very practical point — changing your shoes is something people actually stick to, while a daily exercise routine often quietly disappears after week three.
Curtis et al. (2021), Scientific Reports. Western adults wore minimal footwear for their normal daily activity over six months. Average foot strength increased by 57.4%. Just as interesting: a separate group who had been wearing minimal footwear for around two and a half years had roughly the same strength as the six-month group — suggesting six months of consistent everyday wear gets you most of the available benefit. You are not signing up for a decade-long project.

Why the shoe does this on its own
You are not doing exercises. So what is doing the work? Three design features, all of which have to be present — a shoe with only one of them is not a barefoot shoe.
- Zero drop. Heel and forefoot sit at the same height, so your weight stays centred over the foot instead of being tipped forward. Zero drop shoes let your ankle and arch load the way they were built to.
- A genuinely wide toe box. Toes that can spread act like a tripod. Toes squeezed into a tapered point cannot stabilise anything. This is the feature most often faked — a real wide toe box is shaped like a foot, not like a shoe.
- A thin, flexible sole. Your foot has to bend, grip and respond to the ground. Every micro-adjustment is a rep for those intrinsic muscles.
How long it takes — realistically
This is where people get impatient and get hurt. Strength adaptation is slow by nature; tendons in particular adapt more slowly than muscles.

Where the claim gets oversold
Three honest caveats, because you deserve them before you spend money.
Bigger muscles do not automatically mean better performance. A 2025 systematic review of minimalist footwear training noted that structural changes in the foot do not always translate proportionally into strength or functional gains — particularly in short or low-intensity studies. Your feet getting stronger is real. Your 10K time dropping because of it is a different claim, and a weaker one.
Strength is not a treatment. Stronger intrinsic muscles are widely considered helpful for foot health, but "helpful" is not "cure". If you have a specific diagnosis — plantar fasciitis, a neuroma, tendinopathy — footwear is one input among several, and it belongs in a conversation with your clinician rather than instead of one.
Rushing undoes the benefit. The single most common mistake is wearing barefoot shoes all day in week one because they feel wonderful in the shop. Your foot muscles will cope; your calves and Achilles tendon may not. Mild novel soreness is expected. Sharp or persistent pain is a signal to back off, not to push through.
Who should talk to a professional first
Barefoot shoes are not universal, and any brand that tells you otherwise is selling rather than advising. Speak to a podiatrist before switching if you have diabetes or peripheral neuropathy — reduced protective sensation means a thin sole offers less protection against a cut you may not feel, and a small wound can become a serious one. The same applies to Charcot foot, significant structural deformity, or a foot injury you are still recovering from.
How to actually get the strength benefit

- Make them your default, not your workout shoe. The research measured everyday wear. Ordinary walking, standing and stair-climbing is the stimulus.
- Vary your surfaces. Grass, gravel and uneven ground ask more of your foot than flat pavement.
